THE GRACE DAY SCHOOL SIXTH ANNUAL 5K RUN FOR READING AND ½ MILE FUN RUN In Honor of Jason Massett. Sunday, May 17, 2008 Wantagh Park, Merrick Road. Children’s 1/2 Mile Fun Run: 8:30 AM. Adult 5K Race: 9:15 AM. Check-in begins at 7:00 AM. Day of race registration and check-in will open at 7:00 AM near the start of the race. T-shirts are not guaranteed for race day registration. All registrations must be completed by 9:00 AM.
Full Description
This is a fast and flat 5K that's winds through beautiful Wantagh Park. Race day amenities include: Grace Day School 5K t-shirts Post Race Day Refreshments including: Water, Bagels, Fruit and more... Post Race Raffle and Award Ceremony with the best raffles around..... Children's Magic Show during the 5K Awards to top overall male and female runners, as well as 1st, 2nd and 3rd place in each age category (14 and under, 15-19, 20-29, 30-39, 40-49, 50-59, 60-69, 70 and over) 1st, 2nd and 3rd place awards to top male and female high school runners.

Please accept my entry in the 5K Run (the “event”) on Sunday, May 18, 2008. My fee of $_______ is enclosed. In consideration of my participation in the event, I, for myself, my heirs, personal representatives and assigns hereby unconditionally and forever release, waive, discharge and covenant not to sue any of the sponsor(s) of the event, including, without limitation, Grace Day School, the State of New York, the County of Nassau, the Nassau County Department of Parks and Recreation, the Nassau County Police Department, the Town of Hempstead, Wantagh Park (collectively “Event Sponsors”), and their respective trustees, officers, agents, and employees from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury (including death) that may be sustained by me, or to any property belonging to me, while participating in the event. To the best of my knowledge, I am not aware of any physical disability or health-related reasons or problems that would preclude or restrict my participation in the event. I am fully aware of the risks connected with the event, and I hereby elect to voluntarily participate therein, and to voluntarily assume full responsibility for any risks of loss, damage, or injury (including death) that may be sustained by me, or any loss or damage to property owned by me, as a result of my participation in the event. It is my express intent that this Waiver shall bind my family, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be deemed as a release, waiver, discharge and covenant not to sue the Event Sponsors, and their respective trustees, officers, agents and employees. I hereby grant permission to any and all of the forgoing organizations and instrumentalities to use photographs, videotapes, motion pictures, recordings or any other record of this event for any other purpose whatsoever.
